Uttar Pradesh SIR 2025: What Should You Do If Your Name Is Missing From Voter List Released on 9th December? Here Are Your Options

The UP SIR 2025 electoral list has been released, and now many voters are discovering that their name is not listed. Here is the definitive guide to checking your details, contacting your BLO, filing Form 6 and supporting documents, and making sure you get your name added before ballots are cast.

On December 9, the revised Uttar Pradesh State Integrated Roll (SIR) 2025 was published, with many people checking their names to see if they were on the electoral roll. If your name is not listed, don’t be concerned – the Election Commission has well established procedures to amend the electoral roll prior to the election.

Verifying Online on NVSP or UP CEO Website 

The initial exercise in the process is to verify if it is indeed an error. You can visit the NVSP or UP CEO website, in which you can check your details based on your name, EPIC number or your address. 

Reach Out to Your Booth Level Officer (BLO) 

If your name does not appear, it is best to contact your POLLING STATION Booth Level Officer (BLO). The BLO keeps the list of registered voters and can determine whether your name was removed due to an error, address change, or documentation issue.

Fill out Form 6 to Get Your Name added

To add your name back into the electoral roll, you will need to fill and submit Form 6.

You can apply:

  • Online: through the NVSP portal or the Voter Helpline App.
  • Offline: You can submit the form to your local BLO office or keep it in the Registration Officer (ERO) office.

Attached Required Documents

When you submit the application, Form 6, you need to either upload or attach the following required documents:

  • Proof of identity (Aadhaar, PAN, passport and etc)
  • Proof of address (electricity bill, rent agreement, ration card and etc)
  • A passport-sized photograph.

These documents allow the Electoral Registration Officer (ERO) to verify eligibility for inclusion.

Monitor Your Application

Upon submission, you will be issued a reference number. You may access NVSP or the Voter Helpline App to see updates on your Form 6 status using that number.

If your issue has not been resolved, visit your ERO office

If your issue is still not resolved, go to the ERO office in person. ERO will be able to verify your documents face to face and confirm that your name will be included in the final voter list.
