Respiratory symptoms like wheezing are often quickly linked to asthma, but medical experts are now urging caution. A single sound during breathing may sometimes hide a far more serious underlying condition. In a recent video shared on Instagram, former AIIMS director Dr Randeep Guleria highlighted how doctors and patients should not assume every case of wheezing is due to asthma.

He explained a powerful clinical lesson: not all wheezes indicate asthma. This principle (first mentioned in medical literature by Dr Jackson in the Boston Medical Quarterly) still holds strong relevance in modern medicine. According to him, careful clinical evaluation is essential, especially when symptoms do not improve with standard treatment for asthma.

When Asthma Treatment Fails: A Warning Sign

Dr Guleria shared a case that highlights the danger of misdiagnosis. A woman with breathlessness and wheezing was diagnosed with asthma and started on strong treatment, including oral and injectable steroids. Despite aggressive therapy, her condition did not improve.

Over time, prolonged steroid use led to Cushingoid features, a condition caused by excess steroid exposure. Even after multiple medications and nebulisation, her symptoms persisted, raising doubts about the initial diagnosis of asthma.

On detailed examination in the OPD, a key clue emerged. The wheeze was monophonic, not polyphonic. This subtle difference suggested a possible airway blockage rather than asthma, which usually produces multiple musical sounds in the chest.

Further investigation revealed that routine X-ray and CT scans appeared normal at first. However, a closer look showed a widened carina. A bronchoscopy was then performed, which uncovered a tumour at the bifurcation of the trachea. The diagnosis had been delayed, allowing the disease to progress significantly.

Other Airway Conditions: What Doctors Look For

Experts say distinguishing between different types of wheezing is crucial in diagnosing asthma correctly. Polyphonic wheezing is typically seen in asthma, where multiple airways are narrowed. However, a monophonic wheeze may indicate a single large airway obstruction, which can sometimes be caused by tumours or other serious blockages.

According to a clinical review published by the American Thoracic Society (ATS), persistent wheezing that does not respond to standard asthma treatment should always raise suspicion of alternative diagnoses, including central airway obstruction or structural abnormalities.

While asthma is a common cause of wheezing, it is not the only one. Misdiagnosis can delay life-saving treatment. Doctors and patients should look beyond common assumptions, especially when symptoms persist despite proper care.

In respiratory medicine, careful listening and detailed evaluation can sometimes make the difference between routine treatment and early detection of a serious hidden disease.

UP News: The Uttar Pradesh government has announced that app-powered vehicles would soon be strictly monitored across the state. Under the new guidelines, bike taxis, Ola, Uber, Rapido and delivery vehicles would be required to register online while drivers would need to undergo fitness, medical and police verification tests. The revised regulations are likely to be implemented at the end of this month or next month.

In recent years, dependence on app-based transport has grown exponentially in the country. Concerns about passenger safety, driver accountability and regulatory gaps have also increased. In a major policy shift, Uttar Pradesh under the leadership of CM Yogi Adityanath is now moving to bring these services under a stricter regulatory framework.

New Era of Regulation for App-Based Transport in Uttar Pradesh

Uttar Pradesh is preparing to formally regulate app-based taxi and mobility services across the state under the newly approved Uttar Pradesh Motor Vehicles (Aggregators and Distribution Service Providers) Rules, 2026. The move aligns with the Union Ministry of Road Transport and Highways’ amendment to Section 93 of the Motor Vehicles Act, 1988, which came into effect on July 1, 2025.

Under the revised framework, all app-powered vehicles including taxis, bike taxis and delivery vehicles will be required to register online with the state Transport Department. This marks a significant shift from the earlier system, where aggregators like Ola and Uber operated with limited direct oversight from state authorities.

The policy was approved by the cabinet led by Chief Minister Yogi Adityanath on March 10, and public suggestions and objections were invited until April 5.

How Will It Improve Passenger Security?

The new rules require drivers to undergo comprehensive medical examinations, police verification and fitness tests before being allowed to operate on aggregator platforms.

Every registered vehicle will also be equipped with GPS tracking systems and panic buttons. In case of emergencies, passengers will be able to alert authorities instantly. Additionally, driver details will be displayed on passengers’ mobile phones as soon as a booking is confirmed, increasing transparency and accountability.

A 24-hour grievance redressal cell will also be established to handle complaints, ensuring that passenger concerns are addressed in real time rather than through delayed processes. Another proposal under consideration is the establishment of designated halts or rest zones for app-based drivers. These spaces would allow drivers to rest between trips, improving working conditions and reducing fatigue-related risks.

Historic initiative by Bhagwant Mann Govt, distribution of free food kits begins for 40 lakh poor, Dalit and backward families in Punjab

Punjab Chief Minister Bhagwant Singh Mann on Saturday launched the ambitious ‘Meri Rasoi Yojna’ during a programme organised in village Bhalwan in Sangrur, marking the beginning of free nutritional food kit distribution to nearly 40 lakh poor and economically weaker families across Punjab, as one of the country’s largest household nutrition support initiatives.

Declaring the scheme as a direct intervention to reduce the kitchen burden on struggling families amid rising inflation, CM Bhagwant Singh Mann said the AAP Government is merely making announcements but delivering tangible relief to every household through free electricity, healthcare, women-centric welfare schemes and food security measures. He also asserted that alongside strengthening social welfare, the AAP Government is rapidly modernising infrastructure, with underground power cabling work progressing at speed to make Punjab completely “pole-free” in the coming period.

Addressing a large public gathering during the launch event, CM Bhagwant Singh Mann said, “Previous governments had restricted welfare measures merely to the distribution of wheat and pulses, whereas the Punjab Government has taken a comprehensive step to ensure that kitchens of poor and middle-class households remain functional with all essential cooking items required for daily needs.”

The Chief Minister stated, “Under the ‘Meri Rasoi’ scheme, every beneficiary family will receive a specially prepared kitchen kit every three months containing mustard oil, pulses, sugar, salt, turmeric and other essential grocery items required for running a household kitchen.”

Emphasising that the scheme was designed keeping in mind the practical needs of ordinary families, especially women managing household responsibilities, CM Bhagwant Singh Mann asserted that a strong and prosperous Punjab can only be built when every household kitchen remains functional and no child sleeps hungry. “Proper nutrition for children is extremely important for their education, health and participation in sports. Our Government wants every household to have access to nutritious and essential food items so that children do not suffer weakness due to lack of protein-rich food,” he said.

Highlighting the Punjab Government’s welfare-oriented governance model, CM Bhagwant Singh Mann said nearly 90 percent of households in Punjab are now receiving zero electricity bills, significantly helping families save money and manage household expenses with dignity. “The money saved from electricity bills is now being used by families for household needs and improving their quality of life,” he added.

Speaking about agriculture and irrigation reforms, CM Bhagwant Singh Mann said the AAP Government has ensured uninterrupted daytime power supply to farmers, allowing them to irrigate their fields during the day instead of spending sleepless nights in difficult conditions. “Our government has rejuvenated the canal irrigation system by restoring and laying nearly 14,000 kilometres of water channels and pipelines across Punjab. This historic initiative has improved irrigation facilities and helped conserve groundwater,” he said.

CM Bhagwant Singh Mann further stated that better canal water availability has reduced farmers’ dependence on tube wells, which would play a major role in saving Punjab’s rapidly depleting groundwater reserves.

Reiterating the Punjab Government’s commitment towards women empowerment, CM Bhagwant Singh Mann said registration is underway for the “Mawan Dheeyan Satkar Yojna”, under which eligible women will receive monthly financial assistance. “Women above 18 years of age should come forward and register for these schemes. Women from the general category will receive ₹1,000 per month, while women from Scheduled Caste families will receive ₹1,500 every month,” he stated.

He said these initiatives were aimed at ensuring dignity, self-confidence and financial support for women in managing household responsibilities.

Highlighting healthcare reforms, CM Bhagwant Singh Mann said the Punjab Government is providing free healthcare coverage of up to ₹10 lakh under the Mukh Mantri Sehat Yojna. “People can avail cashless treatment in government as well as empanelled private hospitals. No family in Punjab should be forced to sell land or valuables for medical treatment,” he said.

CM Bhagwant Singh Mann informed that more than 35 lakh health cards have already been issued and the Government is continuously expanding healthcare access across Punjab.

Criticising unnecessary bureaucratic hurdles created in welfare schemes earlier, CM Bhagwant Singh Mann said the Punjab Government has simplified welfare registration procedures by removing cumbersome biometric verification systems that caused inconvenience, especially to women and elderly citizens. “Many women engaged in household work face difficulties in biometric authentication because their fingerprints fade over time. That is why the Punjab Government has simplified the process through photograph-based verification,” he said.

Reaffirming his Government’s commitment towards protecting the sanctity of Sri Guru Granth Sahib Ji, CM Bhagwant Singh Mann said the recently enacted anti-beadbi law has brought relief and satisfaction to people across Punjab. “For years, people suffered pain not only because sacrilege incidents occurred, but also because accused persons often secured bail due to weak legal provisions. The new law includes stringent punishment, heavy fines, long imprisonment and provisions extending up to life imprisonment,” he stated.

He asserted that the legislation has already received the Governor’s assent and there is no question of withdrawing the law. “The overwhelming majority of Sikh Sangat across the world has welcomed the legislation. Only a handful of politically motivated people are opposing it,” he added.

CM Bhagwant Singh Mann further said previous governments failed to take strict action despite repeated public demands, whereas the Punjab Government fulfilled its responsibility by enacting a strong law to prevent sacrilege incidents in future. “His government is working honestly and transparently to ensure that public money is spent on schools, hospitals, welfare schemes and infrastructure development,” he said while thanking people for their overwhelming support and affection towards the Government.

Cabinet Minister Lal Chand Kataruchak, senior civil and police administration officials, panchayat representatives and a large number of local residents were present during the event.

Sharing a few snippets from the event, CM Bhagwant Singh Mann stated on X: “Today in Sangrur, we launched the ‘CM Meri Rasoi Yojna’ for the welfare of 40 lakh families, under which ration kits will be distributed four times a year to protect children from malnutrition.”

“Our Government is making life easier for every section of society by providing free electricity, the ‘CM Sehat Yojna’, the ‘Mawan Dheeyan Satkar Yojna’, uninterrupted daytime power supply and canal water to farmers. From village Satouj, we are also beginning the historic initiative of undergrounding electricity wires to make Punjab completely ‘Pole-Free’,” he added.

“Previous governments merely indulged in politics over sacrilege incidents, but we have ensured protection of Guru maryada by bringing a strict law with provisions of life imprisonment and a ₹50 lakh penalty. This journey towards building a ‘Rangla Punjab’ will continue relentlessly,” the post concluded.

Punjab Chief Minister Bhagwant Singh Mann on Saturday tore into the BJP-led Central Government over repeated Enforcement Directorate (ED) raids on Aam Aadmi Party (AAP) leaders, asserting that Punjab would never surrender before “politics of intimidation and vendetta” being carried out through central agencies. Pointing out that ED had raided Sanjeev Arora’s residence three times within a year and twice within a month without recovering anything, CM Bhagwant Singh Mann questioned why such raids were being carried out only in states where the BJP was not in power.

The Chief Minister pointed out that the Modi Government’s real intention was not to recover black money but to pressure opposition leaders into joining the BJP, citing instances where leaders facing agency action later crossed over to the saffron party and received political patronage. Declaring that Punjab, the land of Gurus and martyrs, had not bowed even before Aurangzeb’s tyranny, the Chief Minister asserted that Punjab would never bow before Prime Minister Narendra Modi either and affirmed that the collapse of the BJP-ED nexus would begin from Punjab itself.

Addressing a press conference, CM Bhagwant Singh Mann said, “Agencies like the Enforcement Directorate (ED), Central Bureau of Investigation (CBI), Income Tax Department and even constitutional institutions such as the Election Commission were no longer functioning independently and are instead being weaponised for political purposes against non-BJP ruled states, particularly Punjab.”

The Chief Minister stated that the BJP had converted these institutions into tools of political coercion to threaten leaders, businessmen and public representatives. “Instead of discharging their duties impartially, these institutions are now being used to settle political scores and destabilise democratically elected governments,” he said.

He said the BJP had adopted the same pattern across the country where raids are conducted not to expose corruption or recover black money, but to pressurise people politically. “The message from BJP is loud and clear: either join BJP or face harassment through raids and investigations,” he remarked.

Citing the recent case of Rajya Sabha MP Ashok Mittal, CM Bhagwant Singh Mann said raids were conducted for two days at his residence, university and business establishments. “But the moment he joined BJP, the raids stopped and he was instead provided security cover by the Centre. What does this expose if not the political misuse of agencies?” he questioned.

He asserted that such pressure tactics would never succeed in Punjab. “We have faced hostile circumstances since our inception. Punjab has never bowed before intimidation and never will,” he said.

Referring to the ongoing ED action at the residence of Cabinet Minister and businessman Sanjeev Arora, the Chief Minister stated repeated raids had yielded nothing. “These politically motivated raids are only meant to create fear, build pressure and arm-twist political rivals,” he stated, while reiterating that Punjab would not succumb to such tactics.

Cautioning Prime Minister Narendra Modi, CM Bhagwant Singh Mann said, “Punjab is the sacred land of Gurus, saints and martyrs. This land did not bow before Aurangzeb’s atrocities and it will not bend before Modi either.” He added that the end of the “unethical alliance” between the ED and BJP would begin from Punjab itself.

The Chief Minister further questioned why ED raids are conducted primarily in Opposition-ruled states while BJP-ruled states remain untouched. He pointed out that since the AAP Government passed the strict anti-beadbi law, BJP had become rattled and had intensified its attempts to intimidate the state leadership.

“For the third time in a year, raids have been conducted at the residence of Sanjeev Arora. Nothing was found earlier and nothing will be found now,” he asserted.

CM Bhagwant Singh Mann said Punjabis had compelled the Modi Government to repeal the three black farm laws and now the Centre was attempting to take revenge by harassing, defaming and targeting Punjabis. “The purpose of these ED raids is not recovery, but political pressure to make leaders join BJP,” he said.

The Chief Minister asserted that Punjab has a glorious legacy of resisting oppression and defending democratic values. “Pressure politics may work elsewhere, but Punjab will never bow before threats or coercion,” he added.

CM Bhagwant Singh Mann said Punjab has historically defeated tyrannical forces and its people know how to protect their rights and dignity even under adverse circumstances. He further stated that the Centre has continuously discriminated against Punjab on crucial issues including Chandigarh, Bhakra, river waters and Panjab University. “The Centre has also withheld Punjab’s rightful dues, including RDF funds and several other pending amounts worth thousands of crores, thereby jeopardising the development of Punjab,” he said.

The Chief Minister said it was unfortunate that despite Punjab’s unparalleled contribution towards national food security and border security, the state continued to be politically targeted. “Punjab feeds the nation by contributing massive quantities of wheat and rice to the national pool, while Punjabi youth continue to defend the country’s borders with unmatched courage and sacrifice,” he stated.

Strongly opposing divisive politics, CM Bhagwant Singh Mann said Punjab’s social fabric was built on communal harmony, brotherhood and mutual respect. “Anything can grow on Punjab’s fertile land except hatred. Attempts to divide Hindus and Sikhs for political gains will never succeed here,” he asserted.

Taking a firm stand on the anti-beadbi law, the Chief Minister reiterated that the Punjab Government remained fully committed to protecting the sanctity and respect of Sri Guru Granth Sahib Ji and all religious scriptures. He said the recently enacted anti-sacrilege law was introduced solely to ensure strict punishment for those attempting to hurt religious sentiments and disturb peace.

CM Bhagwant Singh Mann said the anti-beadbi law had received overwhelming support from sangat across the world. He alleged that only “one particular family” was opposing the legislation because of its own involvement in past sacrilege incidents. “Those claiming that the Panth has rejected this law must explain why millions are supporting it,” he asserted, adding that certain vested political interests were trying to mislead people for narrow gains and create unnecessary confusion around the legislation.

Making his Government’s position clear, the Chief Minister asserted that the anti-beadbi law had already come into force after due constitutional process. “The Governor has granted assent to the Act. There is no question of withdrawing or rolling back this law,” he said.

CM Bhagwant Singh Mann stated that the legislation was enacted after a long-standing public demand and there could be no compromise on protecting the sanctity of Sri Guru Granth Sahib Ji.

The Chief Minister further pointed out that protests and demonstrations had continued for nearly one-and-a-half years demanding a stricter anti-sacrilege law. He noted that several Sikh intellectuals and scholars opposing the law today had themselves earlier demanded stringent legislation against sacrilege incidents.

He also recalled that in July 2007, the Shiromani Gurdwara Parbandhak Committee (SGPC) had itself passed a resolution authorising the government to enact legislation for the protection and respect of Sri Guru Granth Sahib Ji.

Questioning the Opposition’s changing stand, CM Bhagwant Singh Mann asked how those who had earlier supported legislative intervention were now claiming that the government could not enact such a law. He noted that such leaders are trying to run a “parallel authority” and politicising a sensitive religious issue.

Responding to questions on his ongoing “Shukrana Yatra”, CM Bhagwant Singh Mann alleged that ED raids, notices and controversies were deliberately being orchestrated alongside the yatra to divert media attention from the massive public support it was receiving across Punjab.

The Chief Minister also criticised the BJP’s “dictatorial mindset”, pointing out that the BJP wants to control every sphere of public life, from institutions to culture and public discourse. He warned against any attempt to push Punjab towards fear or instability and asserted that Punjabis would give a united and democratic response to such politics.

Taking to X, CM Bhagwant Singh Mann stated, “The BJP-led Central Government is undermining the constitutional framework of the country. The BJP is continuously misusing institutions like the CBI, ED, Income Tax Department and Election Commission for its political interests.”

“Why are ED raids being conducted only in those states where the BJP is not in power? This is a direct murder of democracy. Politics of hatred and discrimination is being carried out against Punjabis. New tactics are being used every day to intimidate and threaten us. I want to make one thing absolutely clear: Punjab is the land of Gurus and saints. In this fertile soil, every seed can grow, but the seed of hatred can never take root here,” he added.

“In reality, the BJP has become completely rattled after the passage of the strict law against sacrilege. Stop playing such games with the people of Punjab.
